2011 Ten Mile River Medical Form

The new 2011 medical form for Summer Camp is now on line – there is a new attachment form to comply with the New York State Department of Health that MUST accompany the national health form.  The additional form from the DOH must be submitted at camp for ALL SCOUTS AND STAFF UNDER THE AGE OF 18!  (Please forward this to ALL Staff and Troops that you may know!!)

Those in camp age 18 or over, still only need just the regular National BSA class 3 medical.

The rules for the meningitis form still remain the same, as usual. (see below).
